CD Jackets - spine printing?

I was wondering if anyone knows how to print spine covers for cd cases....the 
ones that go on the BACK of the jewel case... 

Also....does anyone know if you can purchase the perforated paper for that, 
or if I would have to print and cut myself? 

Any help would be greatly appreciated,
