boxes

What games didn't come with cardboard boxes? I can only really think of 
the double-case cd games(SimEarth) that wouldn't have come with them, 
were there some other of the later cd games they quit making boxes for 
as well?

Also, does anyone have a scan or pic of the TGCD box at all? I've never 
seen on at all, I sure would like to see one, just a silly request :-P
